2020 TRANSFORMING CARE AWARDS

Virtual Conference on LGBTQ+ & HIV Health Equity

Each year at Transforming Care we honor people and organizations who have made significant contributions to the health and wellness of LGBTQ+ and/or HIV communities in the Midwest. In 6 different categories, we will recognize individuals and organizations across the spectrum of care from providers, to advocates, to trailblazers. This is your chance to nominate people and organizations that you feel have created significant and impactful improvement(s) in the field of LGBTQ+ health and wellness, while also working to end health inequities created by racial, economic, ability, and/or language barriers to care. You may vote for as many nominees as you’d like, and you can vote in as many categories as you’d like. Please direct questions to Equitas Health Lead Trainer Ramona Peel at ramonapeel@equitashealth.com.

All submissions are due by Sept 11, 2020
